Study Data

 There are 2 options for reporting Study data:

1. Online data entry:  Login on Physician Login page and enter data online as per instructions.  Enter data for each treated eye separately, and report initial exam and follow-up examinations separately.
2. Download forms and FAX to us:  Download forms by clicking buttons on the left side of the Home Page.  Complete form for each treated eye, and complete forms for initial exam and each follow-up examination.  FAX completed forms to 732 219 9557.
